Like all tyre valve extenders, it's not recommended to leave these on permanently  as they upset the balance of your wheel and put extra strain on the valve stem. This model has a 9mm hex at both ends - many have this on the base only

Makes it much easier to grip with your fingers to screw & unscrew it!

Usually you can put these on and off just fingertight to check tyre pressure
